Our Social Prescribers work with patients to focus on what matters most to them, understanding how life circumstances can affect health and wellbeing. Social prescribing is an effective way to support patients by addressing the social, economic, and cultural factors that can impact mental and physical health.
If you would like to use this service, please contact our reception team, who can refer you to the social prescribing team for support.
Social prescribing can include many things, such as help with:
- Managing debt
- Finding a job or housing
- Accessing support at home
- Carers support
- Volunteering
- Veteran support
- Befriending and lunch clubs
- Leisure activities, getting outdoors and exercising
- Gardening/animal care
- Crafts groups
- Singing
Anything else available through local charities, organisations, community or voluntary groups.
Social prescribing is for anyone who would like to:

- Get information and advice about housing, benefits, employment or training
- Live better with a health condition
- Improve health and wellbeing.
